Summary

A modified version of the Medicare HOS-M is a cross-sectional survey of a nationally-representative sample of the beneficiaries enrolled under the Medicare Advantage plan or Medicare managed care (HMO) plan.  This survey is administered to vulnerable Medicare beneficiaries with the greatest risk of poor health outcomes. The main goal of the HOS-M is to assess the frailty of the population in these health plans in order to adjust reimbursement rates for this population. These beneficiaries are enrolled in the Program of All-Inclusive Care for the Elderly (PACE). It determines physical and mental health at one single time point without any follow up component.

The Medicare HOS-M contains two core components: the Veterans RAND 12 Item Health Survey (12 physical and mental health status questions), and Activity of Daily Living (ADL) items (6 items to compute frailty adjustment factor). In addition there are items related to physical activity levels, functional status, memory loss, and urinary incontinence. The survey has the option of completing responses using self-reporting or proxy reporting.

Medicare advantage plan (HMO) beneficiaries enrolled in Program of All-Inclusive Care for the Elderly (PACE) organizations

Prior to 2014:  1,200 beneficiaries were randomly selected if the plan consisted of at least 1,400 individuals, and all eligible beneficiaries were included for plans with fewer than 1,400.

2014 onward:  1,200 beneficiaries are randomly selected if the plan consists of at least 1,200 individuals, and all eligible beneficiaries are included for plans with fewer than 1,200.

Strengths and Limitations
Strengths Data set employs a combination strategy of data collection (administrative and survey). Adequate and appropriate data documentation available. Oversampling of vulnerable population enrolled under Medicare advantage plan.
Limitations Data not currently publicly available. Data collection is limited to only those individuals enrolled under Medicare advantage plan (HMO plan) thus excluding all enrollees on Medicare fee-for-service plan. Cross-sectional nature of data collection limits ability to follow individuals over time. Survey does not collect socio-demographic variables

The Rehabilitation Research Cross-dataset Variable Catalog has been developed through the Center for Large Data Research & Data Sharing in Rehabilitation (CLDR). The Center for Large Data Research and Data Sharing in Rehabilitation involves a consortium of investigators from the University of Texas Medical Branch, Cornell University's Yang Tan Institute (YTI), and the University of Michigan.
